How we protect your information

Plain-language summary of how the Assured Hearing OWCP intake handles your data. Final legal language is reviewed by counsel.

You are anonymous until you choose otherwise

Before you enter contact details, we only store the answers you give — no name, no phone number. You are always in control of when you share more.

Consent is recorded, exactly as you saw it

When you agree to the HIPAA notice or to receive messages, we save a copy of the exact wording, the time, and which boxes you checked. Transactional appointment texts are separate from any optional updates.

Your information stays in the United States

Compute, database, and backups are US-hosted. Protected health information is encrypted, and every staff access to your record is logged.

Honest routing — no phantom appointments

If we can book you a real time in your area, we show it. If not, we tell you plainly and place you on a waitlist with a follow-up promise. We never show times we cannot honor.

What we store, and for how long

Contact details, eligibility answers, consent records (with the version you agreed to), appointment times, and an activity log. A lead who never books or joins the waitlist is deleted after 90 days.

The Data Promise

After the questions, you may optionally allow de-identified hearing measurements to be used for research. It is never required. You can withdraw later, and later extracts exclude you.
